Consultation Request Form

If you are interested in booking an appointment with me but feel like you need to discuss your idea further to flesh it out, you may benefit from a consultation. This is a free 30-60 minute conversation, either in-person at 444 Tattoo or virtual (via text), and will include in-depth conversation about your concept, as well as placement, style, budget, and anything else you may be uncertain about.

Note: This is NOT the place to request a tattoo & is NOT required to book! It is an optional service that I offer as a courtesy to clients who want to be more involved in the planning process. Disclaimer: I have no problem with making time for an in-person consultation, but I am much more likely to have sooner availability for a virtual consultation.

Please list 3-5 specific dates and times that would work for you within the next month. Please only list dates that align with my availability (see checklist above). Plan for at least an hour time slot. Note that I do not do consultations on Saturdays.

BY SUBMITTING THIS FORM, I ACKNOWLEDGE MY UNDERSTANDING THAT I DO NOT HAVE AN APPOINTMENT UNTIL ASH REACHES OUT TO ME & THAT I MUST ABIDE BY ALL RESCHEDULING / CANCELLATION POLICIES IF I NEED TO CHANGE MY CONSULTATION TIME (SEE FAQ FOR DETAILS). I UNDERSTAND THAT NO-SHOWING A CONSULTATION, INCLUDING NOT BEING REACHABLE FOR A VIRTUAL CONSULTATION TIME, MEANS THAT I MUST PAY A $50 FEE FOR ANY FUTURE CONSULTATIONS AND THAT THIS WILL NOT GO TOWARDS THE COST OF A TATTOO. I UNDERSTAND THAT SUBMITTING THIS FORM DOES NOT GUARANTEE AN APPOINTMENT AND THAT ASH HAS A RIGHT TO DENY ANY CONSULTATION REQUEST AT HER DISCRETION.
