Custom Tattoo Request Form

This form is for requesting a CUSTOM tattoo. If you are looking to book an appointment for a FLASH tattoo or one of my PRE-DRAWN designs, you can submit those requests here.

PLEASE DO NOT SUBMIT THIS FORM IF YOU ARE NOT READY TO PUT DOWN A DEPOSIT. I cannot hold a date on the calendar for you without one.

I require a deposit of $100 to secure an appointment time and compensate for my time spent designing the tattoo. This deposit goes towards the final cost of your tattoo. This deposit is NON-Refundable, even in the event of an emergency. I accept deposits via Venmo, Cashapp, and Paypal, or you can pay in person with Cash. You can find my payment information here.

Please be as specific as possible, for example “right upper forearm” instead of “arm”. If you don’t know where you want this piece, you can say that!

For reference, a credit card is about 2″ x 3.5″.

**Important Note: I will NEVER copy another artist’s tattoo. If you are submitting a photo of another tattoo, please know I will not be basing my design off of it. Any tattoo reference photos should be for style purposes only. For example: “I like the colors,” or “I like the pose”.

I will never design a tattoo based on any AI-generated images. Any requests to do so will be denied.**
