Custom Tattoo Request Form

This form is for requesting a custom tattoo. To avoid any confusion, please read my FAQ page carefully before submitting a request form.

If you are looking to book an appointment for a FLASH tattoo or one of my PRE-DRAWN designs, you can submit those requests here.

I require a deposit of $75 to secure an appointment time and compensate for my time spent designing the tattoo. This deposit goes towards the final cost of your tattoo. This deposit is NON-Refundable, even in the event of an emergency. I accept deposits via Venmo, Cashapp, and Paypal, or you can pay in person with Cash. You can find my payment information here.
